To prepare for your first visit, simply create an online account where you will provide us with some information about yourself before your appointment. Registering online will also give you secure online access to your health information which is referred to as MY CHART.

We accept most insurance plans and will gladly process your claim. Our front desk is happy to assist you in answering questions that you have about your insurance. It’s also important that you check with your insurance provider prior to your appointment to verify your coverage and benefits.
Insurance policies generally cover only a portion of the total treatment cost. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.
Financing is also available through Care Credit.
48 hour cancellation policy: If you are unable to keep your appointment, we require 48 hours notice so that other patients on our waiting list may be served, and you can avoid incurring any charges for the missed appointment ($25 per 1/2 hour of reserved appointment time).
